Unlike a simple Price Quote (which can vary slightly), a formal PEO Proposal provides a locked-price from a PEO based on a review of your company's information. Of course, the Proposal is free and requires no further obligation on your part. All forms and documentation should include your business name and be faxed to (321) 249-0513.

To obtain a formal PEO Proposal please complete our online Pricing Request or download and fax a hard copy. In addition to your Pricing Request, please take a moment to fax us the following:

1. Three years of of Loss Runs - This report needs to be current within last 30 days and is available from your Workers' Compensation carrier or insurance agent. If you are a new business or if Worker's Compensation Loss Runs are unavailable, please complete and fax the following Loss History Affidavit [click here].

2. Copy of Workers' Comp Dec Page - This is the Declaration Page from your current Workers' Compensation policy.

3. Health Census - If your company would like current PEO health rates please complete and fax the following 1 page Health Census [click here]. For accurate pricing analysis please be sure to also fax a copy of your most recent Health Insurance premiums if you currently offer health.

4. Copy of current Payroll Register - If you currently use a Payroll Company or PEO please fax a copy of your most recent payroll register.
